Fowler's Mill Golf Course on
Monday, June 11th
10:00 AM - Registration/Putting Contest begins
11:00 AM - Shotgun Start
Conclusion of Round: Reception / Dinner

Cost:   $150 per golfer (includes lunch & dinner)
             $600 per foursome (includes lunch & dinner
             $150 for a Hole Sponsor (signage at tee area at hole)
             $20 per event packet (includes: 1 ticket each for: Prize board, Lottery 
                    Tree, Putting Challenge & 50/50 Raffle)

• Gold Sponsor: $1,000 (includes 4 golfers, industry exclusive, company info in welcome bag, name/logo on PR materials, representative at hole (optional), link to your website from ours, provide welcome bag give-aways), lunch & dinner
• Silver Sponsor: $600 (includes 2 golfers, company name on PR materials, representative at hole (optional), link to your company's website from ours, provide welcome bag give-aways, lunch & dinner

When registering online, your acknowledgment is also your credit card receipt.  Please note names of those golfing with you, if known.
